Full disclosure: This is not the greatest case ever. I knew there was a good chance this would look crazy if we tried to close the space (and it kinda does if you're into unwavering Golden Proportions). The primary driver of the treatment was root caries, not vanity.
I'll answer a couple questions preemptively:
Yes, he is on Prevident for life......yes, he should have had ortho, among other things....yes this is immediate post op marginal photography......yes, I got permission to share (not sure if Dolores signed off). BUT the email we received made this case worth sharing with you all, if for no other reason than to make you laugh.....

Sheila please tell Dr. Vacek and his lovely aides (who did an excellent job of not waterboarding this mouth breather) that I could not be happier with the results of my last two visits. I now smile wide without self-conscience inhibiting me, I’m back to being the wise guy, smart a s s I’ve always been, yet without the embarrassment of my yellow gap teeth.

My Delores is 69, I’m 66. I like older chicks, she liked younger guys.... I really do believe your work served to end the, uh.... 14 year “drought” my Delores and I had. 45 years of joy reignited. Mmm, mmm, mm.
I’ll see you again in a few months for a regular cleaning and work on my last cavitous back tooth.

Craig, Great result! If you are unhappy with this result, you need to step back a bit and look again.

@Marc: You are right....I was totally thrilled, but mostly with the patient's response...It was just kind of crazy because I told him up front that I wasn't going to try to close the diastemma entirely due to risk "fat, wide tooth syndrome". Wasn't until I was goofing around in design and thought "hey, this might work!"

Great work Craig! Yes, super tough situation that you were presented but great result!!! You may get a phone call from the Pharm reps saying you are taking away from their business a little by enhancing their love lives now with your dentistry!

Very nice work. That is one hell of a transformation. How did you handle the deep bite? Adjust the lower anteriors?

You should post the .rst file for someone to play with to see how hard/easy this can be. Great design skills and way to make his smile look natural! I vote that this .rst becomes the new Level 2 final exam for students so they can see how they can do this at home!

Nice results. You should be happy with what you did for him. But gross, kind of like my parents or grandparents telling me they still have sex. That's something you can't unthink.
Case looks awesome, should be very proud. From the sounds of the letter he is a funny pt and i would imagine fun to see and work on. Great attitude. "Get your biteguard in Delores, its go time"

That is a fantastic life changing result. I'm not heavy into golden proportions, and you actually are pretty close. About the overbite, I've placed plenty of anterior crowns in situations with similar overbites. Sometimes it is necessary to take a little bit from the lower incisors to keep from making the linguals of the maxillary incisor crowns too thin. It is difficult to get enough lingual reduction for maxillary incisor crowns when the overbite is that deep. I always prepare the patient for that possibility in advance.

The caries and existing restorations dictated these would be traditional, non cingulum saving preps, so the minimal thickness on the lingual was a bit of a challenge as I recall, especially on the lateral, so it was going to be Emax for sure, at least in my hands. We did not need to adjust the lowers, outside of some minor polishing.
